Buen día Dr. Steve
Mi pregunta va encaminada a lo siguiente:
Actualmente mi conteo de CD4 es de 68 con una carga viral de 75000 y aun no he iniciado el tratamiento antiretroviral. Ademas de ello se me han desarrollado condilomas intrarectales y en la zona anal debido a que tengo VPH. Dedibo a que me estan afectando mucho tengo programada una cirugia para su extirpacion. Cuando me programaron la cirugia mi conteo de CD4 era de 243 y el inmunologo autorizó la intervención.
Ahora con este conteo tan bajo de CD4 es posible hacer la operación?, o es mejor realizar la operacion posterior al inicio del tratamiento antiretroviral?, Que me recomienda Dr. Steve?

Buen día,
Es definitivamente posible hacer la operación como programada. No necesitas esperar hacerla hasta que inicies el tratamiento anti-VIH. Sin embargo, con respecto al esperar hacer algo más (es decir el inicio de tratamiento), con un recuento de CD4 de 68, no hay ninguna razón para no pensar seriamente en empezar tratamiento. También, con un conteo bajo como el tuyo, ya deberías estar tomando la profilaxis de la neumonía por pneumocystis carinii (PCP, por sus siglas en ingles) y también de la toxoplasmosis, si tuvieras anticuerpos (IgG) en sangre contra el Toxoplasma gondii, el parásito responsable de causar toxoplasmosis.
Bueno, ahora tienes mis recomendaciones. Más que previsto, ¿sí? Mucha suerte y un gran abrazo.
