Que tratamiento debe administrarse para la diarrea causada por isospora, su Cd4 estan por debajo de 150, esta recibiendo trimetroprin + sulfametoxazol, mejora unos dias y vuelve con las diarreas, esta deshidratado, y bajado bastante de peso. Hay algun medicamento para ayudar a que aumente su recuento de Cd4?

Buenos días,
Trimetoprima-sulfametoxazol (TMP-SMZ) se considera como fármaco de elección del Isospora belli. La dosis recomendada es una tableta forte (160/800 mg) cada 6 horas durante 10 días, seguida de dos veces al día durante 3 semanas. La respuesta de esta infección caracterizada por diarrea y pérdida de peso (pero no fiebre) a TMP-SMZ suele ser buena e incluso espectacular. Tratamientos alternativos incluyen pirimetamina más ácido folínico o Cipro. A propósito, una vez que se recupera, si su recuento de CD4 persiste por debajo de 200, necesitará continuar tomando TMP-SMZ una tableta tres veces a la semana.
En cuanto a tu pregunta: "¿Algún medicamento para ayudar a que aumente su recuento de CD4?" sí, hoy en día hay más de 20 fármacos antirretrovirales (anti-VIH) entre los que se puede escoger para construir un régimen eficaz, duradero y, se espera, relativamente bien tolerado. ¿Está contando tu amigo con los servicios de un médico especialista en VIH? Si no, debe hacerlo inmediatamente. Si ya está en manos de un especialista, él o ella deberían poder formar una buena terapia. Un recuento de CD4 más alto le ayudará a tu amigo a recuperarse de la diarrea.
