PROBLEMA EN HIGADO y RETROVIRALES

Ante todo gracias por la respuesta.
Dr. Steve reciba un saludo cordial desde Perú. y paso a detallar el problema.
Tengo un familiar que le detectaron el VIH el 2005, el en la actualidad tiene el CD4 = 15 por lo que los médicos me dicen es que debería entrara al tratamiento TARGA (programa para la toma de retrovirales) pero aún no se lo dan por que tiene un problema en el Higado y ellos dicen que no puede iniciar tratamiento mientras el higado esté inflamado (fosfatasa alcalina = 1700 y en aumento) quisiera su opiníon doctor por uqe se que esta en los niveles demasiado bajos si valdria la pena arriezgar.

Hola Luís,
Claro que con un recuento de CD4 de 15 células, tu familiar es un candidato definitivo para el tratamiento TARGA (tratamiento antirretroviral de gran actividad). Sin embargo, es bien posible que calmar la inflamación hepática sea lo primero a este momento y sea necesario antes de empezar el tratamiento contra el VIH. No puedo decir definitivamente porque sé muy pocos detalles sobre el "problema en el hígado" de tu familiar. No es poco razonable retrasar el tratamiento TARGA algunas semanas mientras se están ocupando de la inflamación hepática. Empezar el tratamiento es muy importante pero no emergente. Lo que es aún más importante que el tratamiento antirretroviral al corto plazo, a la luz de su enfermedad por VIH y su CD4 bajo, es la profilaxis de algunas infecciones oportunistas, en concreto la neumonía por Pneumocystis carinii, el complejo Mycobacterium avium (MAC) y tal vez la infección por Toxoplasma gondii. También debe tener un examen oftalmológico para descartar retinitis por Citomegalovirus (CMV).
