Hola gracias por toda la informacion que es de gran ayuda para aprender y saber sobrellevar esta carga que a veces se vuelve pesada. Tengo la inquietud de saber si puedo mezclar combivir y efavirenz con licor de vez en cuando unas 4 cervezas, Y si es posible mantener relaciones sexuales con mi pareja discordante en los dias fertiles para lograr un embarazo con poca posibilidad de contagio.

Hola,
Por lo general, en cuanto al consumo de alcohol junto con un régimen antirretroviral, todos los excesos son malos. Beber no es nocivo, si se hace con moderación.
En cuanto a tener un hijo con tu esposa negativa, el método más seguro se llama "sperm washing," por la cual la esperma está separada de los fluidos y las células del semen que puedan llevar el virus. Entonces, la esperma puede ser usada para la inseminación artificial o la fecundación "in vitro." La esperma sí misma puede llevar VIH, pero la mayoría del VIH está llevada por las otras células del semen. Deberías hablar a tu médico especialista en VIH de esta técnica y de su disponibilidad en tu área. Si no pueden utilizar "sperm washing," hay varias cosas que pueden hacer para minimizar (pero no eliminar, por supuesto) el riesgo:
1. Alcanza una carga viral indetectable y mantenla por lo menos seis meses antes de mantener relaciones sexuales con tu pareja.
2. Mantén relaciones sexuales con tu pareja solamente en los días fértiles.
3. Habla con tu médico especialista en VIH de la posibilidad de utilizar la profilaxis pre-exposición (PrEP, en sus siglas en inglés). Es decir que tu pareja tomaría un régimen de antirretrovirales antes de mantener relaciones sexuales para disminuir el riesgo de transmisión del VIH.
