Querido doctor Steve
Un saludo para ti, desde la hermosa Colombia. Mi pareja es mujer y es portadora del HIV, yo soy negativo. Tengo una pregunta, hemos tenido 3 relaciones donde no ha habido penetración, solo sexo oral con condon de ella hacia mi, y le he realizado estimulacion manual con mis dedos. Yo acostumbro a comer unias por mi problema de ansiedad y podria en teoria tener microlesiones. Ademas al succionarle los senos, senti leche materna, porque ella recientemente (hace 4 meses) tuvo un nino, cuya prueba se le realizara proximamente.
Mis preguntas esencialmente son:
1)Estoy en riesgo de contraer el VIH de esta forma?
2)En que edad se puede tener un resultado definitivo de un test Elisa para un bebe recien nacido.

Querido Salomón,
Basado en la descripción de tu exposición potencial sexo oral protegido, estimulación manual de ella con tus dedos (incluso con la posibilidad de "tener microlesiones" de "comer unías") y succionarle los senos (aún sentir un poco de leche materna) el riesgo de la transmisión del VIH es bien bajo. Existe un riesgo teorético si la leche maternal hubiera hacer contacto con una herida abierta en la boca o si el fluido vaginal hubiera hacer contacto con una herida abierta de un dedo, pero la probabilidad es poca. Si te preocupes, hazte la prueba. También te aconsejo que dejes de comer unías.
El test ELISA no sirve bien en un bebe recién nacido a una madre positiva porque los anticuerpos (y no necesariamente la infección) se pasan desde la madre al bebe durante el embarazo y así lo más probable es que el Elisa del bebe recién nacido sea positivo y que persista así por muchos meses. Por lo tanto, un test llamado PCR, por sus siglas en inglés, es recomendado para diagnosticar el VIH en un bebe recién nacido a una portadera, y esta prueba es generalmente muy precisa a los tres y seis meses después del parto. Si el resultado del examen es negativo a los seis meses y el bebe no fue amamantado, una infección por VIH es sumamente improbable.
