viramune+combivir Zaigen?
Apr 11, 2001

Dear doctor I am now taking viramune+combovir with a lot of experience in the nuke class(vl undectetable). If VL starts to increase on this combo should I add ziagen or add a PI? what do you think?

Response from Dr. Boyle

If the viral load becomes detectable, I would check a genotype/phenotype (if its high enough) and base my decision on those results. If you experienced a low-grade viral breakthrough wher3e those tests are not available(between 50-1000), I would consider adding abacavir, but given your heavy experience in the NRTI class, this may not be very effective. Adding a PI (since you're heavily NRTI experienced, I presume you are also PI experienced), also may not work due to underlying resistance and I would not add that on to a failing regimen.
