Just diagnosed,Cd4=105, VL=389,734. Doctor put me on 1xSustiva/1xTruvada combo(taken together, once aday)+co-trimoxazole. Im still waiting for hep results but started treatment 5 days ago. What if Ive got liver disese? would that treatment make it worst? Do you think this is the best treatment for me? Since my Hiv is so advanced will it still work ? What are the chances that my VL go down to undetectable level? Please try to answer asap. thank yoy ever so much

Your HIV regimen is a very good choice in the absence of any pre-existing drug resistance. The odds are good (> 75-80% in most studies and my experience) that your viral load will become undetectable if few/no doses missed. That combination (efavirenz + Truvada) is generally well tolerated by persons with liver disease and actually if often a good treatment for hepatitis B if that is active for a given patient co-infected with HIV as well. Good luck! KH
