Response from Dr. Henry
Good questions. The cause of fat loss in face is complicated but age, genetics, time with HIV, history of low CD4 count can contribute as well as the HIV drug used. I assume you have been switched off thymidine analogue drugs such as stavudine and zidovudine? Reversing severe fat loss is difficult partly due to lack of fat cells that might have been lost in the process. Besides plastic surgery interventions, use of some meds (particularly some diabetes meds) remain under study as does use of NucleoMax for treatment of fat loss (jury still out as to benefits and long term safety). Whether some of the newer classes of meds might have less of a problem (such as integrase and CCR5 inhibitors) is yet unclear but I am hopeful that their effect of fat status may be minimal to beneficial. Newer meds under study generally have their effect on fat evaluated with a neutral or beneficial effect on fat being a positive factor in drug development. I can't say that I am aware of any drug in development that will clearly be a major step forward particularly for persons with already established facial fat loss.
