increasing health issues
Jul 7, 2009

I have been HIV+ (known) for about 18 years and on antiviral meds about as long. I have had increasingly worsening health problems of late: congestive heart failure, low platelet count (@30) diagnosis ITP ,WBC (@2.8), poor liver function (SGOT @70, MCV & MCH slightly elevated) CD4 @280, CMV AB IGG @22.0, enlarged lymph nodes, cluster headaches, night sweats and fatigue....sorry to be so long. I have a fairly good handle on the CHF but; I am concerned about the low platelet counts (I do have ongoing bleeding from my nose and mouth), the CMV and the continuing night sweats and strong fatigue. My doctor, who I really like and trust, seems to want to take a wait and see approach (I have a separate cardiologist); but the bleeding (my lower legs are completely discolored) is of strong concern to me; and as so many of us know the night sweats and fatigue are a pain. Any fresh ideas as to how to approach my health issues?

Response from Dr. Henry

You don't mention which HIV meds you are taking (this is a forum for side effects of HIV treatment). ITP can be managed in some cases with effective HIV suppression. Other cases may require courses of corticosteroids, sometimes gamma globulin or splenectomy is needed to manage severe cases. Many HIV+ patients seem to tolerate low platelet counts fairly well but if you are having easy bleeding then more aggressive Rx may be needed (often need involvement of a hematologist). KH
