 Increased belly fat is a complex issue in the setting of treated HIV infection and often not easy to manage. Many non-treatment HIV factors can contribute (age, time with HIV, low CD4 nadir) as can non-HIV factors (such as family genetics). His current regimen has a variable and not well defined effect on abdominal fat. There may be some benefit from switching off the Combivir to Truvada or Epzicom and off the ritonavir/saquinavir to raltegravir or a NNRTI such as nevirapine or efavirenz but that is highly variable and may not be appropriate depending on past treatment history. Exercise and diet can help modestly and is recommended. Drug treatment for increased intraabdominal fat is still of uncertain cost/benefit so no specific regimen can be generally recommended.
