Optimal management of lactic acidosis depends on the underlying cause with withdrawal of offending agents and supportive care key factors. Use of IV NaHC03 requires careful monitoring and titration with the goal to raise the blood pH to 7.20 and the plasma HCO3 level to 8-10 mEq/L. The situation is more tricky if there is renal impairment in which case dialysis with a HCO3 based dialysate sometimes is needed.
