In your opinion, what is the best treatment for neuropathy?
| Response from Dr. Henry
The best approach to HIV treatment related neuropathy is to avoid it in the first place by using HIV drugs that are not associated with it. Monitoring for neuropathy and switching off possibly offending drugs is another important approach. Stopping offending drugs is another important aspect of dealing with treatment-related neuropathy. HIV itself (and other conditions) can also cause neuropathy so another important principle to to try and pinpoint the cause of the neuropathy and then tailor therapy accordingly. For established neuropathy Neurontin in high doses is a popular treatment. Judicious use of non-steroidal drugs and narcotics is also often required. I often involve neurologists and pain specialists to maximize the relief that can be offered. Some advocate acupuncture, massage, or alternative therapies (see Jon Kaiser's book Healing HIV or www.daair.org website for some ideas)--I have had variable success with the latter approaches. KH
