Re: Anal Fissures/Anal Bleeding
Oct 2, 2000

Keith Henry, M.D.: "It actually is unusual for the antivirals to aggravate a yeast infection..." ----- Chronology of 'Immunorestitution Disease' in Patients Receiving HAART Clarified ----- Then why did this get published? ----- WESTPORT, Aug 04 (Reuters Health) - The term "immunorestitution disease" best describes the paradoxical clinical deterioration experienced by some patients with AIDS as their CD4+ lymphocyte counts improve during highly active antiretroviral therapy (HAART), according to researchers in Hong Kong. ----- That term expresses "the concept of immunosuppression followed by a 'bouncing back' of the immune system," Dr. Kwok-yung Yuen and colleagues from Queen Mary Hospital write in the current issue of Clinical Infectious Diseases, dated June 2000.

Response from Dr. Henry

A small percentage of persons experience an "immune rebound" syndrome during the first months on effective therapy. Occasionally that involves response to a fungus (more often to other germs such as mycobacteria). It is a paradoxic situation since it reflects a rediscovered immune system responding to a germ that should not be growing. There actually are lots of reports of that happening (the current issue of the Annals of Internal Medicine has several) and doesn't translate into not using the drugs but rather being aware that it can happen. KH

Keith Henry, M.D.
