Why is my CD4 count lagging?

Dr. Henry, I'm just curious about why some individuals have such a laggard CD4 response compared to others. I just read a post from someone who had a CD4 jump from 40 to 400 in 18 months. In those same 18 months, mine has gone from 78 to 163. We're both on Atripla and both undetectable. Obviously, there may be a million slight differences between me and him, but I just don't get why I'm still in the "slow lane" compared to everyone else.

CD4 recovery depends on a number of variable besides effective control of HIV replication. Some of the key factors identified have included age (older patients often recover CD4 less), degree of residual thymus gland function, degree of scarring of lymph tissues throughout the body, and genetics. Unfortunately for many of those variables no clinically available test can determine likelihood of CD4 recovery which is another reason why starting HIV treatment earlier (at higher CD4 count) often is a good ideas to at least stabilize the CD4 count in the event actual CD4 recovery is slow.
