fuzeon + amikacin = Pain!!!

Hi docs

My partner begain tx for MDRTB two months ago with a regimen that includes amikacin, ethambutol and other second-line tb drugs.

Two weeks ago she began a new ART after a year off of HIV drugs. Her new regimen includes Fuzeon, prezista+norvir, abacavir and tenofovir.

The daily IM amikacin shots are a nightmare for her. She can barely walk due to the pain. Now on top of that she is injecting herself with the fuzeon, which isn't SO bad compared...

are there pain control strategies, or better yet, what alternatives to amikacin can you all imagine

Amakacin can be given IV ( a hassle but may avoid pain from IM injections) if that is an option. 2nd line MAI drugs present many challenges with amakacin often a key component so not easy to find effective alternative drugs in many situations. KH
