My girlfriend was giving me a handjob and she used her fingertips on the head of penis, around the urethra opening. she was being very gentle and wasnt doing it vey hard. Ive read that fluid can only enter if its some sort of rough activity like sex. I'm a little concerned because she didn't wash her hands prior to the handjob. I'm a little worries that if her hands had blood on them from any source (touching a door or something along those lines) it would transmit HIV. Is this a real risk or am I just over thinking ?

There is absolutely no risk from the situation you're talking about.
Even if she had blood on her hand, it would be way too old to infect you with HIV.
Blood typically has to leave someone's blood and go pretty quickly inside of someone else's body. That didn't happen in your case.
