I'm a HIV negative male and my wife is a HIV positive female. We are having a baby together and everything is going fine so far- 8 1/2 months along. So last night we're having sex and after we finished I pulled out to find that I had some blood from her vagina on my penis. I went into the bathroom and washed off in the shower immediately and made sure to pee and push out any possible blood in my urethra but I wanted to know what you thought my risk might be? When we had sex I had just went down on her and no blood so it must have just happened during the sex which didn't last very long so she couldn't have been bleeding long.

You don't mention if you were having protected (latex condom) sex or not. If you were using a condom and managed to get some blood on your penis that did not get under your foreskin or into your urethra, your risk would be negligible. If you were having unprotected sex or your wife's blood and cervical secretions did get on your foreskin or into your urethra, you are at much greater risk. If your wife's HIV plasma viral load is undetectable, the transmission risk would be significantly less. Since this occurred last night, I would suggest you call your wife's HIV specialist immediately. Once he is able to further define your actual risk, he will advise whether a course of PEP (post-exposure prophylaxis) is warranted. Don't delay. PEP needs to be started as soon as possible and no later than 72 hours after exposure.
