PEP effectiveness and response time

Sep 30, 2006

dear dr. bob, i've become interested in pep after starting it to treat a broken condom situation. i want to ask whether any quantitative work has been done about how the effectiveness of pep is related to the time it was begun, ie like a graph of the effectiveness. can we get an idea also about how the virus migrates after infection. you gotta understand i think about little else. thanks

Response from Dr. Frascino

"Quantitative work" cannot be done, as there are way too many variables route/type of exposure, amount of inoculum, immune integrity of host, viral strain, medications used for PEP, etc., etc., etc. What I can tell you is that the sooner PEP is started after an exposure, the more effective it is. Consequently, starting PEP within two hours is better than starting it at 24 hours, which is better than starting it at 48 hours, etc., etc., etc. PEP is not recommended if more than 72 hours has elapsed since the exposure.

Good luck! You're doing everything you can for the moment. Perseverating on or worrying about efficacy will not change the reality of the situation.
