An otolaryngologist-psychiatrist working in addiction medicine who now has an interest in HIV patients??? Wow, that's quite a combination! I'm quite certain you'll keep all of your various skills well honed by seeing HIV-positive patients. You'll probably also find helping HIVers to be both an enriching and rewarding experience.
