Response from Mr. Kull

By front and back I'm assuming that you are saying that you put your mouth on your partner's penis (fellatio, blowjob) and his anus (usually known as rimming). Rimming someone poses very minimal risk for HIV infection, as long as blood isn't present. For this reason, it is probably safer to not put your mouth on your partner's anus following anal sex when blood is more likely to be present. Rimming someone does put you at risk for Hepatitis A infection, intestinal parasites, and possibly other infections of the genital region.

Performing oral sex on an HIV positive man would be riskier than rimming him, but unprotected oral sex is much less risk than having unprotected anal sex. There is evidence that people have been infected by performing oral sex on a man, although the numbers seem pretty low. Your risk for infection is probably increased by getting semen in your mouth and/or having poor oral health (cuts, sores, infection).

If you would like to decrease your risk for infection when having oral contact, you can use a latex barrier to prevent oral contact with semen, feces, and blood. If you choose not to use a barrier, try to keep as much fluid as possible out of your mouth (don't let your partner ejaculate in your mouth) and make sure you have good oral health at the time.
