How Important is resistance Testing in New Infections

Hello Doctor!

How important is it that people with recent (past 12 months) infection get testing for resistance. Are there a lot of resistant strands out there? I read that at a rcent conf. there seems to be growing concern, but it doesn't seem to be standard of care to automatically do genotype (sp?) testing. This I don't understand becuase you generally only get a few shots with current medications. So wouldn't it make sense to make the first shot as best as possible?

It depends on whether you have a high number of already infected people in your community and a high rate of new infections. We are using the test in my clinic for newly infected, drug naive patients. This is distinct from newly diagnosed people, who may have been infected for quite some time and not known it. The likelihood of drug resistance in them is extremely low. MH
