Should I be concern about loosing weight?

Dear Dr. Nelson,
I'm 22 years old, 5'7 180lbs that I know I've maintained and on medication. Atripla to be exact.
My last doctors appointment was on 6-11-08, and my cd4 count came up and is now 379 @ 27% from 373 @ 22%, and a viral load to undetectable.
I've lately been having diarrhea and been feeling very ill with nausea and feel like vomiting. I stop taking my centrium vitamins after I called my pharmacist and they told me to see if that stops it and it stopped my feeling of vomiting, but, I've been noticing that I've been loosing weight.
I went down to 178 from 180 in 2 weeks but I been eating regularly and haven't felt sick.
If I continue to loose weight at what point should I call my doctor and find out what's going on? lets say if I go down from 178 to 173 should I call my doctor immediately or before then?
I'm not on any diet and I've actually been eating well except with some diarrhea, but I can tolerate it. Should I be concern?
Also, if I continue to have any persistent nausea with my medication is it something I should call my doctor about?
I don't have another doctors appointment until 12-11-08, but I'm just worried. I've taken my medication every day and done really well. I haven't vomited even though I felt nausea.
