Dear Dr., I am a very paranoid person as you can see. Like the title implies, will a person catch HIV the first time that they have unprotected sex? Or can they get lucky and not catch it?

If you have unprotected sex, then the risk of HIV infection depends mostly on whether your sexual partner is HIV positive. Since you have no sure way of knowing your partner's HIV status, then the safest thing to do is to always use condoms when having sex.
