The early symptoms of HIV-associated dementia are apathy, social withdrawal, fatigue, and slowed thinking. However, these symptoms are not very specific, and the presence of one or more of these symptoms does not mean someone has dementia.
In order to reach a diagnosis of HIV-associated dementia, persistent symptoms of memory impairment, other cognitive impairment, and neurological symptoms like hand tremor, weakness and gait instability are also usually present. In addition, these symptoms need to be of sufficient severity and duration to cause substantial functional impairment, ie. inability to work, impairment in social functioning, etc. In order to firmly establish a diagnosis of dementia, a medical and neurological evaluation also needs to rule out other common causes of cognitive impairment, like opportunistic infections of the brain, brain tumors and other medical/neurological conditions that may masquerade as dementia.
