Dr. This might be a stupid question. But i want to know if im HIV positive could i have children? My viral load is undetectable and my T cells 364. Thanks...

This question is both very simple and very complicated. The simple side is that the chances of your being able to have children is unaffected by HIV infection. The complicated part is whether it's a good idea. Semen is known to transmit the virus. However, taking anitretrovirals decreases the viral load in semen and decreases the risk of transmission (but does not decrease it to zero). Therefore, inseminating a woman with your sperm puts her at risk for HIV infection, and also puts the child at risk for infection. The risks can be minimized with antiretrovirals, but not eliminated.
