Some of your questions do not have precise answers, but here goes.

The time duration of exposure during vaginal sex in order for HIV to cross the male urethra is not known. We can say that vaginal sex poses a signigicant risk to the man and the woman inolved, if no precautions are taken. The efficiency of infection is much higher in male to female than female to male transmission. The presence of genital ulerations increases the risk of transmission, but is certainly not a requirement.

The lowest chance of infection is associated with using barrier protections (condoms, etc.) during sex. This cannot be said often enough.
