Response from Dr. Horwath

Trazodone is an antidepressant that is metabolized by an enzyme called CYP450 3A4. This enzyme is inhibited by some protease inhibitors, especially ritonavir. This means that taking these 2 drugs together could result in higher than expected levels of trazodone.
When using these drugs together, it may require adjusting the dose of trazodone. However, trazodone is mostly used in low doses for sleep, and with this type of use, it is unlikely that a drug interaction would result in any problems. Typically, when used for sleep, trazodone is given in doses of 50-100 mg at bedtime. When used together with ritonavir, it may result in being more sedating than usual. In that case, reducing the dose to 25-50 mg at bedtime would be appropriate.
