i suspect that my brother has HIV, i accidentally saw a HIV case manager form in his things and I was shocked, i love him , if he wants to keep this secret i'm here for him because most of all, I love him and disappointed that he could not tell me this. Should I confront him if so how, what do I say? Please help.

I believe your brother is best served by respecting his privacy. He must have his reasons for keeping this to himself. When he feels ready, he is perfectly capable of telling you. I believe that "confronting him" will primarily serve your needs rather than his.
