|Low cd4 count

My friend has hiv and his cd4 count is at 37. He is on medication now but not hiv medication he will go on it next month. What happens if his cd4 count goes to 0

Unless there's some overriding reason to the contrary, your friend should already be on treatment. Current international and US HIV treatment guidelines recommend that HIV treatment be started as soon as possible after diagnosis. This is especially critical for people with severe immune suppression (such is the case of your friend).
Waiting to start treatment places people living with HIV and low CD4 counts at risk for further immune decline and high risk for life-threatening cancers and infections. Waiting a month to start with a pre-treatment CD4 of 37 is unacceptable, IMO.
