Undetectable VL and PrEP

Is there really a need of PrEP if the partner is on effective therapy? I was reading that an undetectable viral load puts the transmission risk to almost negligible levels at least for straight couples.

Hello and thanks for posting your interesting question.

If the positive partner is adherent on therapy, and is known to have an undetectable viral load, the risk of HIV transmission is very, very low. Low enough to have a conversation about the risk of transmission and whether PrEP adds any additional risk reduction.

If condoms are used (as they might to prevent pregnancy or sexually transmitted infections other than HIV), I'd think that PrEP would not add significant risk reduction.
