Hello Dr Ben,
I have used 4 or 5 dentists in Thailand, Bangkok, Phuket, and Samui. From my experience it has never been requested to inform of your HIV status, with that said, the clinics that I have attended I have disclosed and have not had any issues and or prejudice. All are extremely clean, wear gloves, shades and masks. The work that they perform is first class and extremely economical.
If concerned, I would always ask on the telephone, and or when you in person make an appointment (with the Dentist) ... That's what I did.
The bangkok dentist I attended is as follows.
