From my experience in Thailand there are excellent private hospitals and clinics in Bangkok, and over Thailand. I have listed a few in order in experience with HIV.

The hospitals below are private and very very good. Thou there is a cost, there are other hospitals as well, being
I would perhaps find a doctor with good English and who you would feel comfortable with. The majority of the hiv medications that would be prescribed would be available as generics in India. I would suggest a referral to a doctor within India, that will be able to continue prescribed meds, and follow up cd4 and viral load tests.
You could then make a yearly visit to the Bangkok doctor for a check up.
Another great clinic is the Red Cross. Thou I am not sure if they do prescriptions and medications. Thou do all blood work.
