what will be the first treatment and medicine for a HIV infected patients?

That is a short but very complicated question.
There is no ONE best treatment for everyone. There are 4-6 "preferred" starting treatments based on guidelines from expert panels (from the US Department of Health and Human Services and the INternational AIDS Society-USA).
These combos usually combine drugs from different classes that will block the virus at different points on it's life cycle.
Determining which is best for any individual will depend on a number of issues, such as:
Is there any drug resistance in your virus?
What other medical problems (such as high cholesterol, diabetes, kidney problems, hepatitis, anemia, acid reflux) do you have?
What other medications are you taking to treat these other problems?
What type of schedule do you maintain (erratic, travel alot, stress level, sleep patterns, etc)?
What allergies or other drug intolerances do you have (screening tests for drug reaactions, such as an HLAB5701 test may be ordered)?
If a female, do you plan to get pregnant?
Do you have trouble swallowing pills or have a preference on number, size and timing of medications?
How good are you at following a schedule to take meds?
Are there certain types of side effects that you would want to avoid if at all possible (GI upset, vivid dreams, rashes, etc)?
So there is no "One size fits all", but we have so many drug combinations to choose from that we can generally find teh combo that fits any individual best.
