meds running sti regime
Oct 13, 2012

due to being abroad travelling just im gonna be 2 weeks short of meds (sustiva and kivexa) before my return to clinic for re-supply. what is the best way to stop for this structured sti. ( i have 2 weeks supply left but i am 4 weeks away from re-supply.)vl-undetect, cd4 500 ..on same tablets for 4 years/no side efects+very healthy. im thinking that sustiva stays in body for 7days and kivexa 3 days so should stop 4 days apart to avoid monotherapy??

Response from Dr. Young

Hi and thanks for posting.

A difficult question for me to answer with any precision, but it's not unreasonable to wonder about the risks of functional monotherapy if one stops medications with different half lives at the same time.

In this circumstance, I'll often just tell patients with undetectable viral loads to simply stop all at the same time, since the worse thing would be to mistakenly continue only on of the two pills. If you're careful not to do this, then it's reasonable to take the Kivexa (Epzicom over here in the States) for a couple of days after stopping the Sustiva.

No mater what, let your care provider know about the interruption in treatment, as the timing of this would affect monitoring labs.

I hope that helps, BY
