I've recently been told that there is another milestone for CD4 levels. Such as CD4</= 200, diagnoses for AIDS 350 = time to consider treatment. Now I'm told that a low level of 40 CD4's = ??? Nobody can give me an answer.

I'd update your definitions. A CD4 count below 350 is abnormally low (below the lower limit of normal); below 200 is AIDS defining.
A count that is 40 is very low, though I wouldn't call it a milestone from a clinical management perspective. Someone with counts this low is a very significant risk for developing advanced AIDS complications.
HIV treatments are now recommended for most people living with HIV, irrespective of CD4 count.
