Response from Dr. McGowan
Thanks for the post. Treatment should be planned for each person, there is no cookbook. That is one size does not fit all. A test for drug resistance should be done first. Assuming that it shows no resistance in your virus to any medication, the current guidelines for first line treatment by the US Department of Health and Human Services would be a choice between 4 equally effective treatments: tenofovir + emtricitabine plus either 1) Efavirenz OR 2) Darunavir with low dose ritonavir OR 3) Atazanavir + low dose ritonavir OR 4) Raltegravir. The choice will depend on several issues such as other medical conditions you may have and medications you may be using your kidney and liver function, the level of your cholesterol, the type of side effects you may/may not want to risk, number of pills and doses you are willing to take, etc. These issues should be worked out with your doctor to pick which (or some other cocktail) would be best for you.
Still worried about possible HIV exposure.
