Thanks for your post. The use of two protease inhibitors (in this case, atazanavir and fosamprenavir) with ritonavir boosting would be for very selected cases where there was significant drug resistance. Combos like this should be used only be experts, since there are a number of issues with drug-drug interactions that could cause unexpected differences in drug levels. Given the availability of more potent PIs, like tipranavir (Aptivus) and darunavir (Prezista), as well as early access programs for the new classes of HIV medications-- CCR5- and integrase inhibitors, the need to use such untested combinations (dual boosted PIs) should be declining. Indeed, in our clinic, there are scant patients remaining on this type of regimen. Hope this helps. BY | |||||||||
