hi dr young. its jose from portugal again.i would like to know your opinion about if we will have therapeutical vaccines not in these days but in some years.will that vaccineeventually make a diference in the way that hiv is treated with current meds.do you thing that a therapeutic vaccine would eventually help people that have started haart with high viral load?or not?these new classes of drugs that will be available(ccr5,etc)do you think that they will have benefits over those classes that we already use?fewer side efects,less resistance,etc? i think that we really are in a point that we are really making great progresses in the figth of hiv. i would like very much to know your opinion about all this questions.

Therapeutic vaccine research for HIV continues, though I have to say that I'm not enthusiastic about seeing one in the near future.
The clear thing (and hurdle) for any new treatment is that current treatments work quite well and are getting increasingly better tolerated. Thus, any new treatment is going to have to meet very high standards for effectiveness and tolerability.
Resistance to any antiviral will remain an issue and area of concern; trying to predict the specific attributes or risks of a new drug is like trying to forecast American politics-- important bt very difficult to forecast.
