psoriasis treatments
Sep 24, 2005

I am currently getting treatment for my psoriasis using the light treatments. I go three times a week for four weeks now and haven't seen any improvements. I am also applying the vaseling ointment. My question is how long should I continue this treatment without seeing improvement and if I do see an improvement how long will it last. Also, my brother also had psoriasis and has been taking methotrexate and his psoriasis is gone. My doctor will not prescribe this medication to me. Why?

Response from Dr. Young

Thanks for your post.

There isn't a unified approach to treating every patient with the same condition (witness the multiple ways that HIV doctors start antiretroviral treatment).

Using UV treatment and topical emolients, like petroleum jelly, are conservative, first-pass treatment for psoriasis. It's safe and inexpensive, and far less side effect prone or toxic than methotrexate (an immunosuppressant). It's because of these later issues that make many doctors reluctant to use immune modulators without a compelling reason to do so.

In my clinic, patients that have serious psoriasis get their treatment from dematologist- or rheumatologist-specialists.

If you've not had any improvement in 4 weeks, you need to speak to your doctor.

Good luck, BY
