|When to Start

Hello, I found out I was positive in 1992 and still not on meds. My last 2 labs were 317, 22% vl 1640 322, 22% 4030 vl These were all in the past 6 months. They have very slowly fallen over the years but I want to start before I get sick. Do you think I can wait much longer or should I start??? I also would like to wait as long as I can. During all the years i've been positive My tcells have never fallen below 300 and my viral load has never been over 5000, usually between 1000-2000.

There's no solid, single answer to your question, but given that your viral load is still quite low and your not reporting any change in your CD4 percentage (and an insignificant change in absolute CD4) I don't see enough change here to prompt initiation of treatment.
I'd probably wait until your next set of labs before making any strong recommendation to start, given your reluctance to initiate treatment. Now, I wouldn't wait too long to get monitoring labs at any time when close to thresholds for starting treatment-- any new symptoms should prompt a call to your doctor too.
