Finding Care when Uninsured

I am a US Citizen residing in an Asian country where HIV care is not as advanced as that in the US and Europe. I am planning a return to the US, but I will be uninsured initially, until I find a new job. What options do I have for finding healthcare in a major city like New York or San Francisco? Please help, as I feel like I've nowhere to turn. Thank you.

There should be good access to care in either New York or San Francisco. I'd contact one of the local AIDS service organizations (www.asofinder.com)for the inside scoop.
