Hi Doctor,

If a person was able to find a combo of antiretroviral drugs that fit their schedule and were able to adhere to this combo, would the combo work indefinately if a single dose was never missed?

Most data demonstrate that once you reach adherence levels above 90% or more, the more adherence the better at keeping HIV under control. In the longest running trial of combo therapy, persons on AZT+3TC+indinavir have remained with viral loads that are undetectable for 7 years. These peopel are very adherent.

So, while absolute adherence does not guarantee a person will have a viral load that becomes and stays undetectable, it increases the chance greatly. Other factors such as the potency of the drug combo, absorbsion of the drugs, perhaps some genetic factors, infection with a resistant virus, also play a role in treatment success. DW
