CD4 count only 23

Sir/Madam, Here is a HIV patient (India) with CD4 count only 23.. Started treatment with Lamivudine&Zudovudine (Duovir Indian Trade name) 20 days back.. Body temperature is not coming to normal.. varies from 100 to 105 Foreign Heat... using Paracetmol 500mg Tables every 4 hours..

Your valuable suggestions needed..

There can be scores of possible explanations for the continued fever. I would be most concerned that the individual has another infection you have not yet diagnosed. A common mistake is to ascribe a fever in a very immunocompromised host with AIDS to HIV itself. HIV can cause fevers but a temperature increase to 105 degrees Fahrenheit would point me in other directions.

Cultures of the blood, urine and stool (if loose) and a chest x-ray would be in order and quick. Empiric antibiotics (including coverage for salmonella) may be needed if the patient is very ill. Good luck - DW
