this email may help the person who asked the question about how quickly can therapy work.
my partner was diagnosed in July 2008 with (like you) a low CD4 count (10) and a VL of 50,000.
He had PCP at time of diagnosis.He is 43.
He was put on Atripla in August 2008 and his numbers look like this.
September 2008; VL 1000; CD4 50
OCTOBER 2008; VL 250;CD4 90
DECEMBER 2008 VL UD;CD4 104
MAY 2009;VL UD;CD4 150
He has never missed his medication.
Like you he showed some signs of resistance (to efivarenz)but doctor said it was a small mutation and started him on it anyway...to see would it wor...and thankfully the resistance did not prove a problem.
