| Should I Switch? Jun 24, 2004 Dear Dr. Moyle, I am on Trizivir and Sustiva and have been for 2 years. Previously, I was on Viracept, Epivir and Zerit. I have had lypoatrophy since 99 and perhaps the reason it hasn't improved is because of the AZT in Trizivir. I do have some Epivir resistance, but that seems to be it. Could I remove the AZT from my cocktail and replace it with something else? Regards, Jeff |

Given the epivir (3TC) resistance you would need to replace the AZT. The choices here would be either tenofovir (Viread) and perhaps ddI (Videx). Tenofovir has not been associated with fat loss so would be a reasonable choice. Replaement of AZT in a regimen has not been particularly well studied but there are some data to support the idea that its continuation in a regimen may prevent fat recovering.
