hemoglobin and platlets

My fiance was diagnosed with HIV 4 months ago and has recently been admitted to the hospital for a completely unrelated reason. For the past 2 days his hemoglobin and platelets have been rather low (as of yesterday his hemoglobin was at 50 and his platelets were at 70,000 even after a transfusion) however he hasn't been bleeding externally so they did a CT scan and could not find any internal bleeding either. What could be going on?

You don't indicate what the current status of his HIV infection is (i.e., CD4 count, HIV viral load, on HIV treatment or not), or whether he has a history of other ongoing medical problems and/or taking medications for those. What you describe are very nonspecific findings which could be the result of many different medical problems, or if this is a case of advanced HIV infection, a result of HIV-associated complications.
