Good Day Doctor, Please help me understand here, I've been hiv positive for 16 yrs and have never been on meds,my VL has over the years always been undetectable, but the CD4 counts are low,now at 198.I don't feel sick or anything ,but what is going on here? my DR wants me to go on Atripla immediately.

Response from Dr. Holodniy

You don't indicate what your CD4 counts have been over the last few years, or more specifically over the last few months. In addition, what is your CD4%, total white blood cell count (WBC) and total lymphocyte count. Are all your white blood cells decreased, or just your CD4 cells? If all your cells are decreased something other than HIV may be going on. On the other hand, if all your other cells are normal and this is a decline of just CD4 cells, despite the undetectable viral load, this could still be HIV and HIV treatment would be advised.
