I am 7.5 months pregnant, expecting twins. I am on ART, and will deliver by cesarean to reduce chances of infecting the children at birth. However, I have a dilemma about whether to breastfeed, or for how long. My Dr says breastfeeding infection can be prevented, but my friends insist it is safer not to breastfeed at all. I am confused!! Please advise.

You don't indicate what country you live in, as the guidelines/recommendations vary, particularly between developed and developing nations. Although women on HIV treatment with undetectable viral loads can safely breast feed, as infection to the infant has been dramatically decreased, there is still the risk of infection to the infant. Thus, the only real way to completely prevent infection is not to breast feed.
