Just diagnosed, my HIV is a very rescent infection(march 2009)...First month test, my cd4 was 670, vl 130000. 2nd month, Cd4 was 350, vl above 500000. My doctor suggests starting therapy, another doctor says "wait there is always this fluctuation initially". I also had a pnemounia vaccine day befor my second test. Any thoughts?

Your viral load is very high and your absolute CD4 count is bouncing around. I would also keep track of your CD4 percent which is a more stable number over time. I don't think there is an urgency to starting meds, but if on repeat your CD4 count is in the 350 range and your viral load is > 100,000, I would recommend starting treatment.
