Lab refuses to do bloodwork

I live in New York State, have been getting bloodwork done at a University HIV clinic about 2 hours away every three months. Asked my HIV provider if I could use a local outpatient walk-in lab the next time, I've been UD last two times, after starting meds early last year. The local lab took my info, looked at the script then refused to do the HIV viral load without a state form filled out. Emailed my provider who says he's not been asked to fill out a state form for years. Is it true a lab will refuse to draw a viral load based just on a valid script in NYS?

I can't speak to your specific case, but labs need to follow the money. If they don't have a contract or method for payment system in place with a physician or hospital, they will not perform the test.
